Getting started with Paymaster Payroll has never been easier. Below you’ll find a list of typical documents we need to get your payroll account started along with links to useful websites. Unsure of where or what some of the documents are? Don’t worry; feel free to contact us anytime for help.
Required Documents
- Federal ID Number
- Legal name as filed with the IRS
- Legal Address
- UBI Number
- Voided check for payroll account
- W-4’s for all employees
- Rate notices for L&I and Employment Securities
(other notices if applicable)
- Year to date report with all payroll balances by employee and company totals
- Prior quarter year to date payroll report with all payroll balances by employee and company totals
(If prior payrolls have been paid this year)
